Psychiatric Care

As the incidence of Alzheimer's and other cognitive disorders increase, so too does the need for psychiatric treatment options. At Avalon we recognize the special needs of patients who utilize psychiatric services. We have developed an integrated model of care that coordinates mental health services with other health care needs. Coordination between mental health staff and medical staff directly correlates with improved patient outcomes.

                In partnership with Valley Mental Health, Avalon has implemented a best practice model for the delivery of mental health services to nursing home residents. We developed a comfortable, secure environment where patients will receive innovative, on-site care from a multidisciplinary team of professionals. The team will generally consist of a psychiatrist, nurse and licensed clinician who provide patients with individualized care to meet their specific needs and condition. Multidisciplinary models have been shown to yield the best result with reduced symptoms and hospitalizations.
