Located in beautiful American Fork, Utah between Utah Lake and Mt. Timpanogos.  We have been serving the healthcare needs of our community since 1984. There is a wonderful history to our building, being the original American Fork hospital. We are located five minutes from the new American Fork Hospital in a nice residential area. 

Residents are our first priority. We offer a continnum of services to address the individual need of each resident. Our unique campus allows aging in place from our Assisted Living, Memory Care Unit to our Skilled Nursing unit.
