Located in the heart of Salt Lake City, Avalon Valley Rehabilitation is a 140 bed Medicare Certified skilled nursing facility committed to providing compassionate and dedicated care to the residents we serve. We specialize in short-term rehabilitation services for those seeking transitional care from an acute medical facility and boast Utah's premier integrated Skilled Behavior Health Program.

Patients are our first priority and we are committed to meeting the physical and emotional need of each individual in our care. We offer a continuum of services to anticipate and meet the individual needs of each resident. No two people are the same. Providing just the right amount of support and care is important. Our comfortable surrounding and personalized care allow residents and their families to feel at home.

The Avalon Valley Behavioral Health Model

Avalon Valley Rehabilitation is a joint venture project of Avalon Health Care and Valley Mental Health, the largest providers of rehabilitation and mental health services in Utah. It is the only non-hospital rehabilitation faclity in the Intermountain West that has comprehensive mental health services located on site to provide for the complete behavioral and mental health needs of their residents.

The relationship between Valley Mental Health and Avalon Health Care began in 2002 and culminated with the opening of the Avalon Valley Rehabilitaiton in 2005.

Psychiatric Services

• Psychiatrist available 5 days a week to provide evaluations and rapid follow up on changing needs
• Onsite coordination of medication management between house Physician and Psychiatrist
• Licensed staff available for individual therapy needs
• Licensed staff available 24/7 for crisis intervention
• Mental Health Staff available 7 days a week to provide psychosocial rehabilitation services
• Mental Health Staff maintain therapeutic milieu and behavior management programs
• Daily coordination of care between mental health and health care staff
