







Situated in the heart of the Northpointe Medical Community in Spokane, Washington, Avalon Care Center at Northpointe is a Skilled Nursing and Rehabilitation Center offering Spokane’s premier subacute unit for short-term rehabilitation. Our Care Center was recently renewed, refreshed, and redefined as of 2012.
Avalon’s team of expert nurses provides 24-hour nursing in a healing environment. In addition, Brighton Rehabilitation’s therapists provide individualized physical, occupation and speech therapies designed to help you or your loved one achieve improved independence, strength, endurance and overall functioning. The same therapy team works within our building every day, so you or your loved one will feel comfortable. Outpatient therapies are also available.
Avalon at Northpointe is the only Skilled Nursing and Rehabilitation Center in Spokane with a dedicated on-site physician who focuses on only one facility on a daily basis. Leornard Vanderbosch MD, our Medical Director has been practicing medicine for over 45 years. Dr. Vanderbosch provides supervision at Avalon Care Center at Northpointe 24 hours a day, 7 days a week to help ensure the highest quality of care for the long term and subacute residents. We are especially blessed to have such a dedicated Medical Director.
Our Leadership Team Encompasses 120+ years of Healthcare experience. The Leadership team includes: The Facility Administrator, the Director of Nursing, the Medical Director, the Director of Social Services, the Director of Community Relations, the Director of Admissions, and the Office Manager.
We have a preventable hospital readmission program called “Keep Me Home”. In 2012 Avalon Care Center at Northpointe averaged about 10% below the 2011 National average of 20%, for hospital readmissions for the same diagnosis/injury. This program helps ensure positive outcome driven care is appropriately accessed.
Our comfort care program strives to put patients and their family members at ease nearing end of life by reducing pain and addressing physical, psychological, social and spiritual needs. The plan helps patients live the best possible quality of life. The comfort care program is similar to hospice programs and is usually covered by Medicare Part A.
